Why is it a good practice of the Person-Centered Model?
It is a strong example of person-centered practice because support is shaped around the individual student’s needs, preferences and goals rather than around standard institutional procedures. The approach ensures that each person with a disability receives tailored accommodations that enable full participation in academic and social life. By promoting autonomy, self-advocacy, equal access and individualized support, it embodies the essential principles of the Person-Centered Model.
Integrated Assessment (Person, Family, Housing) and Life History
The service conducts an individual needs assessment focused on functional abilities, academic requirements, mobility, sensory access and available supports. While it does not collect detailed life histories or evaluate family circumstances, it considers each student’s living situation — including housing accessibility — and personal conditions to recommend appropriate adjustments. This ensures that support is grounded in a real understanding of the person’s context rather than assumptions.
Personalised Care and Support Plan for the Life Project
A personalised support plan is created for each student, offering academic accommodations, assistive technologies, adapted housing, interpreters, teaching assistants, specialised classes, or psychological support depending on individual needs. This plan enables students to pursue their educational goals, build independence, and participate in campus life with dignity and autonomy. It aligns support with the student’s aspirations — not just their disability.
Support groups
Peer support is available through the university’s disabled-students association, which provides spaces for shared experiences, advocacy, social connection and mutual encouragement. These networks foster belonging and emotional support, enhancing well-being and reducing isolation while reinforcing the sense of community.
Case Management and Resource Coordinator
A coordination unit manages all disability-related supports — assessing needs, allocating resources, arranging accommodations, liaising with academic departments, and ensuring continuity. This centralised coordination acts as a form of case management within the university environment, guaranteeing that supports are implemented consistently and adjusted when needs change.

The model has served as a reference for other higher education institutions in Poland.
