Case Management and Resource Coordinator

Interviews are conducted by reference professionals, supported by the coordination team.

To select the reference professional, the resident will first be consulted to identify who they have a closer relationship with. Then, the professionals will be asked about which residents they have a closer relationship with. The goal is to achieve the greatest possible alignment.

The coordination team will provide training and raise awareness about the importance of interventions based on Life History and will support the professionals at all times.

They will monitor the implementation of preferences and interests.

They will detect possible changes in preferences and interests.

Highlined results

Results not yet confirmed but the implementation of this project aims to address the following questions:

. Do residents have expectations for promoting independent living? What types?

. Does understanding Life Histories improve interventions and result in a higher quality of life?

. Evaluate the experience of users, families, and staff after the implementation of the project.

Creation of support networks for professionals

. Life Histories will be recorded in books owned by the residents.

. A software will be developed for access by users, families, and professionals to ensure the continuity of the project.

. Scores on Fumat and Qualid scales, . indicators of well-being for older adults and individuals with dementia
