Why is it a good practice of the Person-Centered Model?
Different minds is a Scottish Government website written in collaboration with autistic people. The aim of this initiative is to promote a more inclusive and neurodiverse society by recognising and valuing differences in the way people perceive and experience the world. The person-centred model is embedded in the initiative and is explicitly reflected in: (1) information for understanding autism is provided in different formats, (2) the content is mostly written in the first person perspective, (3) it offers resources and support for autistic people and the general community, and (4) neurodiversity is reflected throughout the content.
Integrated Assessment (Person, Family, Housing) and Life History
While Different minds project does not carry out integrated assessments directly, it promotes resources and organizations that can offer specialized support to autistic people, their families, and carers. For example, it highlights services such as the Education Rights and Transitions Service and the Student Support Service, which provide assistance tailored to individual and family needs.
Personalised Care and Support Plan for the Life Project
The initiative emphasises the importance of recognising and valuing neurodiversity, encouraging organisations and communities to adopt person-centred approaches. While Different Minds does not develop personalised care plans, it directs people to services that can assist in creating support plans tailored to the needs and life goals of autistic people.
Support groups
Different Minds highlights information on the availability of support groups and post-diagnostic support programmes that empower autistic people in their life plans.
Case Management and Resource Coordinator
The website provides information on organizations that act as resource coordinators, ensuring that autistic people and their families access the necessary services.
Highlined results
The approach is extended to the first 10 territories and extended to at least 50 new territories. This 2nd stage will enable the experimental panel to be extended and the lessons learned from the 1st stage to be refined in preparation for the 3rd.

The project could be extended to other contexts, replicating some of its core ideas: (1) Involving autistic people in the design and implementation of initiatives; (2) Creating educational resources that challenge myths and promote understanding, adaptable to diverse cultural and social contexts; and (3) Strategic collaboration between government, third sector organisations and the autistic community to achieve shared goals.
